Prior to Registering, Please Read Ye Seife Giorgis Metasebia Edir Society is a non-profit organization created to help its members during a time of grief when a family member has passed away. To register, you must be a Canadian Citizen or Permanent Resident of Canada and a resident of Calgary, Alberta. Your membership is only officially activated once you complete this application and pay the required registration fee. Participation is completely voluntary.

I am a Canadian Citizen or Permanent Resident of Canada, and a resident of Calgary, Alberta. I understand my membership is only officially activated once this application is complete and the required registration fee is paid, and that a 3-month waiting period applies during which no death benefits will be issued. I understand the annual membership fee of $300 CDN ($150 by Jan 31 and $150 by Jun 30). Payments after June 30 incur a $100 penalty, and non-payment past the 60-day grace period results in termination. I understand that membership is non-transferable and non-refundable, fees are not prorated, and it is my responsibility to keep my information current. I have read, understood, and agree to the by-laws, policies, and decisions of the Board of Directors. Any misrepresentation may result in termination of membership. I understand the death benefit of $20,000 CAD is paid only to the beneficiary(ies) listed on this form, according to the By-Law and final decision of the Board of Directors. By signing below, I confirm all information provided is true and accurate, and I consent to YEGMES contacting and sharing membership information with my listed beneficiary(ies) in the event of my passing.
