New Member Application Form

Apply to become a registered member. Choose your plan (Family, Single Parent, or Individual), list your beneficiaries, and submit your signed application online.

Register parents or guardians visiting Canada, or update their record when they become a Permanent Resident or Citizen. Complete this form and download a signed PDF copy.

Family Membership Update Form

For members following divorce or legal separation. Acknowledge Article 9 of the Society's Bylaws, then request to remove or transfer the family membership.
